การใช้ระบบสารสนเทศภูมิศาสตร์ในการจัดทำแผนที่การกระจายทันตบุคลากร :

กรณีศึกษา จังหวัดมุกดาหาร

Using a Geographic Information Systems for Mapping the Distribution

of Dental Personnel : A Case Study Mukdahan Province.

บทคัดย่อ

ปัจจุบัน ปัญหาการกระจายทันตบุคลากรในประเทศไทย มีความแตกต่างกันมาก พบว่าเขตตรวจราชการกระทรวงสาธารณสุขเขต 7 มีอัตราส่วนทันตแพทย์ต่อประชากรมากที่สุดคือ 1 : 27,773 เขตตรวจราชการกระทรวงสาธารณสุขเขต 5 มีอัตราส่วนทันตาภิบาลต่อประชากรมากที่สุดคือ 1 : 24,317 ส่วนจังหวัดมุกดาหารมี ทันตแพทย์ต่อประชากร 1 : 31,551 ทันตาภิบาลต่อประชากร 1 : 18,559 ซึ่งมากกว่าเกณฑ์ที่กำหนด คือทันตแพทย์ต่อประชากร 1 : 12,500 ทันตาภิบาลต่อประชากร 1 : 10,000 เกิดปัญหาการกระจายทันตบุคลากรตามกรอบอัตรากำลังกระทรวงสาธารณสุข ไม่สอดคล้องกับปริมาณงาน สภาพพื้นที่ที่รับผิดชอบ และจำนวนประชากรที่ต้องดูแล

การวิจัยครั้งนี้มีความมุ่งหมาย ประการแรก เพื่อพัฒนาแบบจำลองที่สร้างขึ้นโดยใช้ตัวบ่งชี้ 3 ด้าน ประกอบด้วย ด้านปัญหาโรคทางทันตสาธารณสุข(D1) ด้านประชากร(D2) และด้านการเข้าถึงบริการสาธารณสุข(D3) นำไปแปลงเป็นคะแนนมาตรฐาน Z และ T ตามลำดับ จำแนกการกระจายทันตบุคลากรด้วยอันตรภาคชั้นเป็น 4 ระดับ คือ มาก ค่อนข้างมาก ค่อนข้างน้อย และน้อย ประการสองการทดสอบโดยการทดลองนาจำนวนทันตบุคลากรไปดำเนินการดังแบบจำลองที่สร้างขึ้นเพื่อให้ได้คะแนนมาตรฐาน T ที่มีค่าคะแนนใกล้เคียงกันหรือเท่ากัน ซึ่งเป็นค่าที่เหมาะสมต่อทันตบุคลากร 1 คน และประการที่สามเพื่อ เปรียบเทียบการกระจายทันตบุคลากรตามกรอบอัตรากำลังของกระทรวงสาธารณสุขและการกระจายทันตบุคลากรด้วยระบบสารสนเทศภูมิศาสตร์ ด้วยการหาค่านัยสำคัญทางสถิติ

ผลการวิจัย พบว่า แบบจำลองกำหนดเกณฑ์ ดังนี้ D1 x D2 x D3 ส่วนการทดสอบพบว่าค่าเฉลี่ยด้านปัญหาโรคทางทันตสาธารณสุขที่เหมาะสมต่อทันตบุคลากร 1 คน คือ 2,065 คน ด้านประชากรประกอบด้วยจำนวนศูนย์พัฒนาเด็ก คือ 9 แห่ง จำนวนเด็กในศูนย์พัฒนาเด็ก คือ 261 คน จำนวนเด็กอนุบาล คือ 288 คน จำนวนนักเรียน คือ 1,494 คน จำนวนประชากร คือ 8,444 คน จำนวนหมู่บ้านคือ 20 หมู่บ้าน และด้านการเข้าถึงบริการสาธารณสุขประกอบด้วย ระยะทาง คือ 71.14 กิโลเมตร ระยะเวลาเดินทาง คือ 54.21 นาที และการเปรียบเทียบการกระจายทันตบุคลากรตามกรอบอัตรากำลังของกระทรวงสาธารณสุขกับการกระจายทันตบุคลากรด้วยระบบสารสนเทศภูมิศาสตร์

พบว่า การกระจายทันตแพทย์ ตามกรอบกระทรวงสาธารณสุข เรียงลำดับ 1-7 ดังนี้ อำเภอเมือง คำชะอี ดอนตาล ดงหลวง นิคมคำสร้อย หว้านใหญ่ และหนองสูง ซึ่งแตกต่างจากแบบจำลองอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.01 ส่วนการเปรียบเทียบการกระจายทันตาภิบาลตามกรอบกระทรวงสาธารณสุข เรียงลำดับ 1-7 ดังนี้ อำเภอเมือง ดงหลวง ดอนตาล คำชะอี นิคมคำสร้อย หว้านใหญ่ และ หนองสูง ซึ่งเมื่อเทียบการกระจายทันตาภิบาลตามแบบจำลอง พบว่าแตกต่างที่ อำเภอคำชะอี ดอนตาล หนองสูง และหว้านใหญ่ อย่างมีนัยสำคัญทางสถิติที่ระดับ 0.1

โดยสรุป การใช้ระบบสารสนเทศภูมิศาสตร์ในการสร้างแบบจำลองการกระจายทันตบุคลากรได้อย่างมีประสิทธิภาพ สามารถช่วยให้ผู้บริหารนำผลการวิเคราะห์ไปประกอบในการพิจารณาจัดสรรทันตบุคลากร เพื่อกระจายทันตบุคลากรให้ตรงความต้องการในพื้นที่ในจังหวัดมุกดาหารได้ทันที สามารถประมวลผลได้สะดวก รวดเร็ว เห็นภาพแผนที่ที่ชัดเจน เข้าใจง่าย ครอบคลุมสภาพพื้นที่ที่เป็นจริง สามารถแก้ไขปัญหาสัดส่วนทันตบุคลากรกับประชากรให้เหมาะสมกัน มีจำนวนทันตบุคลากรเพียงพอต่อความต้องการรับการรักษาทางทันตกรรมที่เพิ่มขึ้น มีการกระจายทันตบุคลากรในพื้นที่ชนบทและห่างไกลเพิ่มขึ้น

ABSTRACT

The problems of dental personnel distribution in Thailand currently show great differences. It has been found that in Region 7 of the Ministry of Public Health Supervision the ratio of dentists to population is the greatest 1:27,773, in Region 5 the ratio of dental nurses to population is the greatest 1: 24,317. However, in Mukdahan Province the ratio of dentists to population is 1:31,551, and the ratio of dental nurses to population is 1:18,559. These ratios exceed the established requirements of dentists to population ratio of 1 : 12,500 and dental nurses to population ratio of 1 : 10,000. Therefore, problems of dental personnel distribution based on the manpower framework of the Ministry of Public Health occur, and the ratios are not in accordance with work loads, responsible area conditions, and the population to be cared.

The first purpose of this study was to develop a constructed model consisting of 3 indicators : dental disease problems(D1), population(D2), and public health service access(D3). The model was used to find out the values of data representatives by means of median calculation. The needs for dentists and dental nurses were then evaluated by using standard T-scores to find ranks of their needs from adding up standard T-scores on these 3 indicators. And then levels of needs were divided by using stratified interval into 4 levels : high, rather high, low, and very low. A score was determined to each level of classification. The second purpose in testing on the experiment, the numbers of dental personnel were treated as in the constructed model to obtain standard T- scores with close or equal scores which were appropriate for each of dental personnel. And the third purpose of the study was to compare dental personnel distributions based on the manpower framework of the Ministry of Public Health through the geographic information systems by finding out the statistically significant values.

The results of the study were as follows. The model was D1 x D2 x D3. In testing the ratio of dental personnel to the number of dental disease problems were 2,065 people. The population : the number of child health care were 9 place, the number of childen in child health care were 261 people, the number of childhood were 288 people, the number of student were 1,494 people, the number of population were 8,444 people and the number of


village were 20 village. The public health service access : the number of the way were 71.14 kilometer and The number of time were 54.21 minute. For the dentist distribution based on the framework of the Ministry of Public Health, the first 7 top ranks of needs were: Amphoe Mueang, Khacha-I, Don Tan, Dong Luang, Nikhom Kham Soi, Wan Yai and Nong Sung. This was different from the dentist distribution based on the model at the 0.01 level of significance. For the ranks of dental nurse distribution based on the framewrok of the Ministry of Public Health , the first 7 top ranks of needs were : Amphoe Mueang, Dong Luang, Don Tan, Khamcha-I, Nikhom Kham Soi, Wan Yai and Nong Sung, but Amphoe Khamcha-I, Don Tan, Nong Sung, and Wan Yai showed the different needs mentioned at the 0.01 level of significance.


In conclusion, the use of geographic information systems for constructing dental personnel distribution model efficiently could help administrators implement the analysis results in supplementing to consideration on allocation of dental personnel for dental personnel distribution to meet the needs of each area in Province Mukdahan immediately. The results could be complied conveniently and rapidly. The mapping could be seen clearly, easy to understand, and could cover all the actual area conditions. The problems of ratios of dental personnel to population could be solved for appropriateness with adequate dental personnel for the needs for more dental treatments; and more dental personnel distributions could be made in rural and remote areas.
